Happiness​
-
5pt: Your pet avoids spending time with the family, often hiding or showing irritability when disturbed. Some pets may be indifferent to petting, showing no preference for it.
- 10pt: Your pet is present with the family for at least half of the time, without showing irritability. They greet you cheerfully upon your return home.
- 15pt: Your pet shows affection towards you upon your arrival, your family, and familiar animals.

Eating & Drinking​

5pt: Your pet is declining food and water, possibly nauseous or having vomiting or diarrhea. Cats might linger near the water bowl, beside it, or with their heads over it.
- 10pt: Your pet's eating pace has slowed or their interest in food has diminished. They might revisit their meal multiple times before completion.
- 15pt: Your pet's eating and drinking habits are normal.

​Activity

5pt: Your pet cannot walk or stand without your assistance.
- 10pt: Your pet is on pain medication, which provides relief at least 75% of the time. Or your pet can do about half the activities they did when they were healthier or can spend half the time doing their favorite activities.
- 15pt: Your pet is pain free, fully active and enjoys all activities every day.

Ventilation

5pt: Your pet is experiencing severe difficulty in breathing, coughing, or open mouth breathing. If so, please seek immediate veterinary care.
- 10pt: Your pet is displaying intermittent episodes of coughing, wheezing, or a lack of tolerance for exercise that lasts less than two minutes. If so, please consult with your primary veterinarian as their medication may need adjustment.
- 15pt: Your pet is showing no signs of coughing, wheezing, or intolerance to exercise.

Eliminations

5pt: Your pet lies in their urine and/or feces, possibly due to a lack of control over their eliminations or is not able to move after eliminating. They may have a bleeding, foul-smelling, and/or infected external tumor that cannot be kept clean or bandaged. Additionally, your pet may suffer from pressure sores from prolonged immobility.
- 10pt: Your pet might require help to eliminate but does not lie in their waste. They can control their bladder and bowels until assisted. Any external tumor or mass may be present, but remains clean, bandaged, and uninfected. They groom themselves but might need help in certain areas, such as their rear end.
- 15pt: Your pet is independent in urinating, defecating, and grooming. They are free from medical conditions that could cause unpleasant odors.

New & Old Diagnosis

5pt: Your pet has been diagnosed with a medical condition that is unpredictable and susceptible to abrupt turn of events. Or despite being on pain medication, your pet appears to be in pain, indicated by whining, crying, and reluctance to move. It's important to remember that many animals conceal their pain or weakness as a survival mechanism.

10pt: Your pet has a medical condition that may vary over time but is presently stable. With your veterinarian's guidance, you can monitor and adjust treatment as needed. You're aware of the signs to look for, the treatment strategy, and when to seek medical care.

15pt: Your pet is happy, pain free, fully active and enjoying all activities; there are no medical issues beyond routine preventative care.

A score of above a 70 is a happy & healthy pet.
A lower score suggests reason to consider quality of life.
